The good news is that MiWayLife aims to make the MiFuneral claims process as easy as possible so your family can focus on what really matters during a difficult time.
Step 1: Complete The MiFuneral Claim Form
The first step is to fill in the MiFuneral Claim Form. This form gives the claims team the key details they need to process the claim.
To make things easier, you can also download the “How to Submit a Claim” document, which explains exactly how to complete the form and what documents you will need.
Step 2: Gather The Required Documents
Along with the completed claim form, you will need to email a few supporting documents. These help confirm the details of the claim and allow MiWayLife to process the payout as quickly as possible.
You will need to submit:
A completed MiFuneral Claim Form
A certified copy of the death certificate (BI5)
A certified copy of the South African ID of the deceased
A certified copy of the South African ID of the beneficiary or claimant
A Notice of Death (DHA1663) or Notice of Death by a Traditional Leader (BI1680)
Additional documents may be needed in certain situations:
If the death was accidental or unnatural:
A completed ‘Statement by Police’ form
If claiming a stillborn benefit:
Antenatal records or a gestation period chart
The sooner we receive the complete and correct information, the sooner we can begin processing your claim. Please ensure that all documents are certified by a Commissioner of Oaths.
Where To Get These Documents
Some of these documents are official government forms, but families are usually guided through this process.
The death certificate (BI5) is issued by the Department of Home Affairs after the death has been officially registered. In many cases, the funeral parlour or undertaker will help the family register the death and obtain this certificate on their behalf.
The Notice of Death (DHA1663) is usually completed by the doctor or medical practitioner who confirms the death. This form is part of the official death registration process.
If the death takes place in a rural area or within a traditional community, a Notice of Death by a Traditional Leader (BI1680) may be issued instead. This is completed by a recognised traditional leader and submitted when the death is registered.
In most situations, the hospital, clinic or funeral home will guide the family on exactly where to get these documents and help ensure everything is correctly submitted to Home Affairs.
Step 3: Submit Your Claim
Once your claim form and documents are ready, simply email them to:
