To submit a claim with MiWayLife after a policyholder has passed away, you’ll need:

 

  • A fully completed MiWayLife claim form found here
  • A certified copy of the Death Certificate
  • A fully completed DHA-1663 form
  • Certified copies of the ID of the insured life or of all the beneficiaries
  • Three months of bank statements for the beneficiary(ies)

 

Who will receive the payout?

You ultimately insure yourself when you take out a policy in your name. This means when you die, a loved one who has the necessary documents to process the claim will receive the payout. You do have the option of choosing a trusted family member or friend to begin the claims process to handle your funeral expenses should they have all the necessary documents that your insurer will need to process a claim.
