Making A Claim
The claims process may differ depending on your insurer, but for the most part, when claiming due to the passing of the life insured, you’ll be asked to submit a claim form, a completed DHA-1663 form, a certified copy of their death certificate as well as a certified copy of their ID as well as that of all the beneficiaries. These certified copies will have to be stamped by a Commissioner of Oaths. You may also need proof of banking details for all the beneficiaries.
What Is A DHA-1663 Form?
A DHA-1663 form is a notification of death issued after it’s been registered. It can be obtained from the doctor who certified the death; funeral undertakers; the Department of Home Affairs (DHA) or South African Police Service members in areas where there is no DHA office. Once you fill this in and give it to the DHA, you will be issued a death certificate. Remember to keep all your documentation relating to the claim somewhere safe.
The death certificate that you’ll need to provide along with your claim has to either be the original unabridged certificate printed by the DHA or a certified copy. An unabridged death certificate contains much more detailed information than the abridged version.
In The Case Of An Accident
In the case of an accidental or unnatural death, MiWayLife would want a fully completed police report as well as a copy of the Road Traffic Accident Report in case of a motor vehicle accident.
Medical Information
Documentation aside, as a claimant, you may also be asked to include medical information regarding the life insured. For example, MiWayLife’s claim form requests the details of their doctors as well as the circumstances regarding their passing.
