Making a claim on a life insurance policy might seem daunting but MiWayLife strives to make it as simple as possible. This way, your loved ones can focus on what’s important and not have to feel overwhelmed by paperwork during an already stressful time.

How to make a claim

If you have a life insurance policy with MiWayLife, you’ll be asked to submit the following:

  • A fully completed MiWayLife claim form depending on the type of death – natural or unnatural. (You’ll find all our claim forms and guides over here.)
  • A fully completed DHA-1663 form
  • An original or certified copy of the Death Certificate
  • The funeral parlour invoice including its telephone number, physical address and stamp
  • Certified copies of the ID of the life insured AND of all the beneficiaries
  • Three months of bank statements for the beneficiary(ies)
  • Personal medical attendance report completed by the life insured’s attending doctor

Important things you should know

A DHA-1663 form is a notification of death issued after it’s been registered. It can be obtained from the doctor who certified the death; funeral undertakers; the Department of Home Affairs (DHA) or South African Police Service members in areas where there is no DHA office.

Once you fill in the DHA-1663 and give it to the DHA, you will be issued a Death Certificate. This should be the original unabridged version printed by the DHA or a certified copy. An unabridged death certificate contains more detailed information than an abridged version.

When extra information is needed

In the case of an accidental or unnatural death, MiWayLife will also request a fully completed police report as well as a copy of the Road Traffic Accident Report in case of a motor vehicle accident.

You may also be asked to include the life insured’s medical information. For example, MiWayLife’s claim form requests the details of their doctors as well as the circumstances regarding their passing.

Submitting your claim

MiWayLife aims to make the claiming process as simple as possible so you can submit your claim via email and there’s no deadline. By law, life insurers and investment houses have to honour a claim on a policy no matter how long it takes for the policyholder or beneficiary to come forward. Still, it’s always better to submit your claim as soon as possible to avoid things like losing the important documents required to make a claim.

Remember, if you have a life insurance policy with MiWayLife, it’s always a good idea to tell your loved ones how to access the policy’s information as well as your ID to avoid any unnecessary delays.

Understanding the timelines

If your claim is straightforward and MiWayLife has no reason to decline it, most claims are settled within two to three weeks. The Funeral Benefit, however, is accelerated, so you can expect that to be paid out within 48 hours of MiWayLife receiving the required information. This way, your loved ones can give you a dignified farewell without having to worry about how to finance it. In short, it’s the ultimate peace of mind.
